What documents are needed for a 16 or 17-year-old person to apply for a passport?




In general a 16 or 17 year-old person can apply for a passport with the same supporting document than an adult, along with proof of parental awareness. We strong recommend at least one parent or legal guardian appearing with you in person when you apply for your passport. If not, one of the following document must be submitted:


  • A signed letter from one legal parent or guardian with a photocopy of their ID.

  • Proof that the parent or legal guardian is paying the application fees (e.g. parent or guardian name on the check).

  • The passport agent may ask you to submit a notarized Form DS-3053 from legal parent or guardian together with a photocopy of that parent or guardian's ID.
